What is the Symptom Check-up?
The Symptom Check-up is a series of questions about symptoms you may have. It has been designed by the hospital’s Symptom Monitoring Service to give your medical team more information about how you are.
What do I have to do?
When you come to the clinic, a Symptom Check-up Assistant will ask you to complete a short questionnaire. You will use a touch-screen computer to do this and the Assistant will help you.
Do I need a Symptom Check-up?
We will ask you to have a Symptom Check-up every time you come to the clinic. Even if you are feeling well it helps your medical team if you answer these questions. Please inform us if you do not wish to answer the questions and we will let your doctor or nurse know.
Can this help anyone else?
It is important for us to know more about the symptoms people have. We may ask your permission to use your answers for research. If we did this, we would remove any information that would identify you. What symptoms will I be asked about? The Symptom Check-up includes questions about a range of different problems. You may have experienced some of them yourself. These are all symptoms we know are common in people who attend the clinic.
What happens next?
Your answers will be given to your doctor or nurse before you see them.
We may contact you after your appointment to ask you more about your symptoms. Please do not worry if we call you; this is part of our routine service.
